Norman is a 83 y.o. male who is referred to home care physical therapy
with a diagnosis of congestive
heart failure(CHF). Norman is living in his home with the assistance
of his younger sister, Adrian who is 80 y.o.
Subjective:
- Norman reports he does not need to be restrained in his chair, and
would like you to make his sister remove the posey
- Adrian reports that Norman has frequent falls especially at night when
he attempts to walk to the bathroom
- Adrian also reports that it is difficult to remember to give Norman
all of his medications, and the pills are so easy to mix-up
Objective:
- Gait: with a walker for twenty feet, min. assist., large medial- lateral
displacement of COG
- Vital Signs: At rest: Radial pulse is weak and difficult to assess,
BP is 140/60 sitting
